Insurance Policies
As a courtesy to our patients, we will bill your insurance company if it is listed below. Upon receipt of an Explanation of Benefits (EOB), co-insurance, deductibles and any responsible party balances due immediately. You will not be billed for contractual allowances.
It is the absolute responsibility of the insured (member) or the parent using the insurance policy to know their own coverage benefits and eligibility. The insured (or parent present with the child at the time of the visit, if he/she is not the policy holder) will be responsible for applicable co-insurance, deductibles, and "non-covered" services, which include plan-year maximum benefits, preventative (including immunization) services and other plan exclusions.
